IMO, if you've got Healing Touch and/or Heal Area, you epifail anyway. Use RoF.

Watch the field, and keep an eye on the party-bar, For example if 3 Jotun skull-bashers run towards your ele, You know who to PS/Guardian :P

I normally just watch both. If a party member needs healing/proting, I quickly look on the list of members and heal/prot him. I look on the field because a warrior enemy could be targeting me, so I normally just move around.

healing touch, heal area, and mending touch are all HORRIBLE for pve. well, heal area has some application on an mm and mending touch is nice on a ranger, and that's IT.
more generally though...in pve you can pretty much just watch the bars. often, it's hard to click on exactly who you want to on the field, if they're running around and there are enemies and other players (especially minions) in the way. it's a lot more reliable to click on the party window. however, it makes sense to watch the field a bit, because if you see 4 aatxes charging past your frontline and all heading for the same guy, it's nice to have prot spirit on them before they get there. doesn't hurt to take a quick glance at the radar either, occasionally, to see if anyone is overextending or something.
for pvp, you should definitely be paying more attention to the battlefield.

Though not a personal favorite of mine, you will find it on several monk bars, for example: Draw Conditions & Mending Touch comes to mind.
